About This Notice
Coppell Emergency Room values your trust when you seek emergency care. Federal law requires us to safeguard the privacy of your health information and describe our practices. This Notice applies to records created or maintained at: 720 N Denton Tap Rd, Coppell, TX 75019.
We maintain digital patient records and may securely exchange relevant health information electronically with healthcare providers, labs, insurance providers, or others involved in coordinating your treatment.
We must follow these practices while this Notice is effective. We may modify our practices and this Notice. Modifications apply to existing and new health information. The current Notice is available at our facility and coppellemergencyroom.com.
Protected Health Information Defined
Protected Health Information (PHI) is identifiable information about your health, treatment here, or payment for care. This includes medical history, examination results, diagnostic findings, treatment records, and billing information.
Information Uses
Treatment
Health information enables quality emergency care. We share relevant data with physicians, nurses, lab personnel, and clinical staff treating you. When specialized care is needed, we coordinate with other facilities.
Example: A cardiac event may require sharing ECG results and vitals with a cardiologist or receiving hospital.
Payment
Information supports billing—coverage verification, claim filing, and payment resolution.
Example: Claims include diagnostic codes, services rendered, and treatment details.
Operations
Records support quality activities—clinical evaluations, education, compliance audits, accreditation.
Example: Records may be reviewed to evaluate care quality and protocol effectiveness.
Authorization Requirements
Disclosures beyond those above require written authorization. Withdraw anytime by written request to our Privacy Office. Prior disclosures remain valid. Psychotherapy notes, paid marketing communications, and sale of information always require specific written authorization.
Your Rights
Record Copies
Written requests to our Privacy Office. Reasonable fees may apply. Texas law requires response within 15 business days.
Amendments
Request corrections to errors or omissions. Denial occurs if information originated elsewhere, is not maintained by us, or is accurate. Written disagreement may accompany denials.
Restrictions
Request limits on treatment, payment, or operational disclosures, or sharing with family. Most are discretionary. Restrictions for fully self-paid services withheld from insurers must be honored.
Communication Preferences
Specify preferred contact methods and locations. Reasonable requests are accommodated.
Disclosure Accounting
Request a list of disclosures from the prior six years, excluding treatment, payment, operations, and authorized disclosures. First annual request is free; fees may apply for additional requests.
Paper Copy
Obtain a printed copy of this Notice anytime from the facility.
Breach Notification
Receive notification of breaches involving your unsecured information—details, information types, protective measures, our response, and contacts.
Our Responsibilities
Coppell Emergency Room must:

- Provide this Notice of duties and practices
- Follow current Notice terms
- Notify you when restrictions cannot be honored
- Accommodate reasonable communication preferences
Alert you to breaches of unsecured health information
Complaints
File complaints with us or the U.S. Department of Health and Human Services. No retaliation for complaints.
